Introduction: Although health technologies help improve the quality of services, they create ongoing challenges for health systems. Meanwhile, paying attention to ethical, legal, and ethical dimensions can reduce their negative consequences. This study aims to evaluate how Iranian HTA reports address these dimensions and to derive lessons from international best practices to strengthen the integration of HTA into health technology policymaking.
Methods: A two-phase comparative, qualitative study was conducted. In the initial review phase, a census of all HTA reports generated by the Iranian Office of Health Technology Assessment was analyzed. Subsequently, in the comparative phase, eight countries were selected based on their established leadership and maturity in HTA systems. Data from both phases were analyzed using content analysis, utilizing MAXQDA v20 software.
Results: The findings indicate that none of the analyzed reports employed a standardized framework to address ethical, legal, or rights dimensions during the assessment or reporting processes. In contrast, all selected international comparators integrate these dimensions as primary criteria for prioritizing technologies for assessment. Furthermore, in these countries, the utilization of HTA outcomes is either a mandatory or advisory prerequisite for evidence-informed health technology policymaking.
Conclusion: To enhance the efficiency and efficacy of HTA in Iran, it is imperative to systematically embed ethical, legal, and Legal considerations into assessment and reporting protocols. Such integration will facilitate more transparent, equitable, and evidence-informed policymaking, thereby bolstering the scientific credibility and social legitimacy of HTA. Establishing standardized frameworks and defining binding mechanisms for incorporating HTA findings into the policy cycle is essential.
