برنامه پزشکی‌خانواده و نظام‌ارجاع از محورهای بنیادین ارتقای سلامت و عدالت در نظام سلامت ایران است، اما تداوم و موفقیت آن با چالش جدی ماندگاری‌پزشکان مواجه است. جابجایی مکرر و ترک‌خدمت پس از پایان دوره تعهد، موجب ناپایداری مدیریتی، اختلال در تداوم مراقبت، کاهش کیفیت و افت شاخص‌های بهداشتی می‌شود. بخش قابل‌توجهی از پزشکان، نیروهای متعهد خدمت (طرحی) و فاقد‌تجربه کافی در مدیریت هستند که آشنایی محدود با نظام‌شبکه و کمبود انگیزه‌حرفه‌ای، این چالش را تشدید می‌کند. تجربه نشان‌داده که تکیه بر تأمین موقت نیروی‌انسانی نمی‌تواند پایداری برنامه را تضمین‌کند؛ بلکه نیازمند تغییر نگاه از دوره تعهد خدمت به مسیر شغلی پایدار است. تفاوت‌های اقتصادی، فرهنگی و مدیریتی مناطق کشور در جذب و نگهداشت پزشکان نقش مؤثری دارد. در این راستا باید اصلاح ساختار آموزشی و استخدامی، اجرای برنامه‌های تشویقی و پرداخت مبتنی بر عملکرد و طراحی بسته‌های حمایتی متناسب با شرایط منطقه‌ای در اولویت سیاست‌گذاری قرار گیرد. بازنگری در آموزش پزشکی عمومی، تقویت آموزش مهارت‌های مدیریتی و ارتباطی، تأمین امکانات رفاهی و حمایت‌های اجتماعی از الزامات حیاتی در اثربخشی این برنامه ملی است.
