Introduction:
Sustainable pharmaceutical supply chain management in hospital pharmacies is a critical component of healthcare systems and requires a multidimensional approach. This study aimed to analyze the causal structure and interrelationships among factors affecting pharmaceutical supply chain sustainability, considering economic, social, and environmental dimensions in hospital pharmacies in Yazd City, Iran.
Methods:
This applied analytical study was conducted in two phases. First, factors influencing sustainable pharmaceutical supply chain management were identified using a questionnaire developed based on the Supply Chain Operations Reference (SCOR) model, including five dimensions: reliability, responsiveness, flexibility, cost management, and asset management. The content validity of indicators was assessed using Lawshe’s content validity ratio (CVR), and the final indicators were validated by experts in pharmaceutical management and hospital pharmacy services. In the second phase, the fuzzy Decision-Making Trial and Evaluation Laboratory (Fuzzy DEMATEL) approach was used to determine causal relationships and prioritize influential factors.
Results:
Asset management and cost management were identified as the main causal and influential dimensions, whereas reliability, responsiveness, and flexibility were categorized as effect dimensions. At the indicator level, insufficient staffing, weak relationship management with distributors, ineffective performance of pharmacy and therapeutics committees, and inadequate cost–revenue analysis were among the most influential factors. The causal network map demonstrated complex interrelationships among the identified factors.
Conclusion:
This study provides a systematic understanding of causal relationships influencing sustainable pharmaceutical supply chains in hospital pharmacies. Strengthening asset and cost management, improving inventory systems, and enhancing organizational coordination may contribute to greater sustainability, resilience, and operational performance of pharmaceutical supply chains.
