مقدمه: در زمینه پیامدهای اقتصادی برون‌سپاری خدمات در بیمارستان‌های دولتی ابهام‌ها و تناقض‌های زیادی وجود دارد. ازاین‌رو، مطالعه حاضر با هدف بررسی پیامدهای اقتصادی برون‌سپاری در بیمارستان‌های دولتی در ایران انجام شد.
روش­ ها: این مطالعه به‌صورت مرور نظام‌یافته با جستجوی کلیدواژه‌های مرتبط در پایگاه‌های اطلاعاتی داخلی و خارجی PubMed، ProQuest، Scopus،Web of Science، SID، MagIran و موتور جستجوی Google Scholar بدون محدودیت زمانی انجام شد. پس از حذف موارد تکراری و ارزیابی بر اساس معیار ورود و خروج پژوهش، مطالعات مرتبط انتخاب شدند.
یافته­ ها: از 840 مقاله بازیابی شده در نهایت 20 مقاله وارد مطالعه شد. سه نوع برون‌سپاری اجاره‌ای، مدیریتی و مشارکت دولتی-خصوصی شناسایی شد. در پنج مطالعه کل بیمارستان برون‌سپاری شده بود. هیچ‌یک از مطالعات کمی دارای گروه کنترل نبودند. در کل از 51 شاخص استفاده‌شده بود که بیشترین آن‌ها مربوط به درآمد کل، هزینه‌های کل و هزینه‌های پرسنلی بود. میانگین زمان بررسی اثرات برون‌سپاری 2/24 ماه بود. اکثر مطالعات بیانگر تأثیرات مثبت برون‌سپاری از جنبه‌های اقتصادی بود. مهم‌ترین دستاوردهای برون‌سپاری در این پژوهش می‌توان به کاهش هزینه و افزایش سودآوری و مهم‌ترین موانع به وجود هزینه‌های پنهان، تحمیل هزینه‌های اضافی به بیماران، نبود ثبات اقتصادی و ارزیابی نکردن از لحاظ مالی اشاره کرد.
نتیجه‌گیری: نتایج نشان داد که برون‌سپاری در بسیاری از بخش‌ها و خدمات بیمارستان‌های دولتی موفقیت‌هایی بسیاری از نظر اقتصادی داشته است. اما در برخی از موارد برون‌سپاری‌ها سبب افزایش هزینه‌ها شده است. بنابراین، بررسی وسیع‌تر جهت شناسایی عوامل تأثیرگذار و پیامدهای منفی برون‌سپاری و راهکارهای مقابله با آن باید موردتوجه مدیران بیمارستان و سیاست‌گذاران سلامت قرار گیرد.
