fa امکان‌سنجی پیاده‌سازی پزشکی از راه دور در مراکز درمانی شهر ساوه: مطالعه مقطعی در سال 1402 Feasibility study of telemedicine implementation in Saveh city healthcare centers: a cross-sectional study in 2023 فن آوری اطلاعات سلامت Health Information Technology پژوهشي Research <div style="text-align: justify;">مقدمه: پزشکی از راه دور با وجود محدودیت‌های زیرساختی، نقش مهمی در بهبود دسترسی به خدمات سلامت در مناطق محروم ایران دارد. شهر ساوه نیز با چالش‌های مشابهی روبه‌رو است؛ ازاین‌رو، پژوهش حاضر با هدف ارزیابی امکان‌سنجی پیاده‌سازی پزشکی از راه دور از ابعاد مختلف در مراکز درمانی این شهرستان انجام شد. روش‌ها: این مطالعه مقطعی با رویکرد کمی در سال ۱۴۰۲ در مراکز درمانی منتخب شهر ساوه انجام شد. جامعه پژوهش شامل ۷۰ پزشک، ۱۳ نفر از کارکنان فناوری اطلاعات و ۱۹ مدیر و رئیس مراکز درمانی بود که به روش در دسترس انتخاب شدند. ابزار گردآوری داده‌ها پرسشنامه‌ای محقق‌ساخته مبتنی بر ابزارهای مهرآیین و کیبرو بود که سه بعد آمادگی انسانی-حرفه‌ای، فنی-ارتباطی و سازمانی-مدیریتی را ارزیابی می‌کرد. روایی صوری و محتوایی توسط پنج متخصص تأیید و پایایی با روش آزمون-بازآزمون (r=0.71) تعیین شد. تحلیل داده‌ها با آمار توصیفی و آزمون آنالیز واریانس یک‌طرفه انجام گرفت. یافته‌ها: میانگین نمره نگرش کارکنان فناوری اطلاعات، مدیران و پزشکان به ترتیب ۳.۸، ۳.۷ و ۳.۱ بود که تفاوت معناداری داشت (0.05P<). بیش از ۸۴ درصد شرکت‌کنندگان، نبود بودجه مجزا و کافی و همچنین چالش‌های بیمه و بازپرداخت را مهم‌ترین موانع پیاده‌سازی پزشکی از راه دور عنوان کردند. نتیجه‌گیری: با وجود نگرش مثبت و آگاهی مناسب نسبت به پزشکی از راه دور، ضعف زیرساخت‌های فنی و ارتباطی، چالش‌های بیمه‌ای و کمبود آموزش از موانع اصلی اجرای آن در شهر ساوه است. تقویت زیرساخت‌ها، آموزش مستمر و اصلاح سیاست‌های بیمه‌ای برای استقرار موفق این فناوری ضروری است.  </div> <div style="text-align: justify;">Introduction: Despite infrastructural limitations, telemedicine plays a crucial role in improving access to healthcare services in underserved areas of Iran. The city of Saveh faces similar challenges; therefore, this study aimed to evaluate the feasibility of implementing telemedicine across multiple dimensions in the healthcare centers of this city. Methods: This quantitative, cross-sectional study was conducted in selected healthcare centers in Saveh in 2023. The study population consisted of 70 physicians, 13 information technology staff, and 19 managers and heads of healthcare centers, selected through a convenience sampling method. Data were collected using a researcher-made questionnaire based on the Mehraeen and Kiberu instruments, assessing three dimensions of readiness: human–professional, technical–communication, and organizational–managerial aspects. Face and content validity were confirmed by five experts, and reliability was assessed using the test–retest method (r= 0.71). Data analysis was performed using descriptive statistics and one-way analysis of variance (ANOVA). Results: The mean attitude scores toward telemedicine among IT staff, managers, and physicians were 3.8, 3.7, and 3.1 (out of 5), respectively, with a statistically significant difference (p < 0.05). More than 84% of participants identified the lack of dedicated and adequate funding, along with insurance and reimbursement issues, as the main barriers to telemedicine implementation. Conclusion: Despite positive attitudes and adequate awareness of telemedicine, inadequate technical and communication infrastructure, insurance-related challenges, and insufficient training remain the primary obstacles to its implementation in Saveh.
