بهبود فرایندهای بخش اورژانس بیمارستان با استفاده از شبیه سازی رایانه ای

<b>مقدمه:</b> بخش اورژانس با مسائل گوناگونی مانند کمبود تخت، ناتوانی بیماران در دسترسی منظم به مراقبت سرپایی، و شلوغی و ازدحام روبرو است، که در نتیجه افزایش زمان انتظار بیماران، افزایش مدت اقامت بیماران برای دریافت خدمت و کیفیت پایین خدمات را در پی دارد. هدف اصلی این پژوهش بهبود فرایندهای جریان بیمار در بخش اورژانس است. <br><b>
روش کار:</b> این پژوهش از دیدگاه شیوه، توصیفی و از دیدگاه هدف، یک پژوهش کاربردی است. جامعه آماری پژوهش کلیه افراد مراجعه کننده به بخش اورژانس بیمارستان مورد بررسی بودند که به صورت نامحدود در نظر گرفته شدند. برای مدل سازی فرآیندهای جریان بیمار، از شبکههای پتری رنگین، و برای شبیهسازی و بررسی سناریوهای بهبود، از ابزار CPN Tools استفاده شد. دادههای ورودی مدل شبیهسازی از طریق زمان سنجی در طول زمان جمعآوری دادهها به دست آمده اند. زمانهای انتظار بیماران، طول مدت اقامت و میزان استفاده از منابع به عنوان معیارهای عملکردی تعریف شده است. برای بهبود در فرایندها چهار سناریوی بهبود دهنده (B,C,D,E) تعریف و اثر این سناریوها بر معیارهای عملکردی بررسی شد. <br>
<b><br>یافته ها:</b> خروجی شبیه سازی نشان داد که سناریو B، مدت زمان انتظار برای پزشک متخصص را 45 درصد کاهش میدهد و سناریوهای دیگر اثر مثبتی بر کاهش زمانهای انتظار نداشته اند. سناریو E بیشترین کاهش در طول مدت اقامت بیماران سطوح مختلف ESI داشته است. دو سناریو E و D بهبودهای بیشتری در استفاده از منابع داشته اند.
<br><b>نتیجه گیری: </b>در انتخاب سناریوی برتر به مأموریت و رسالت اصلی بخش اورژانس، یعنی نجات جان بیماران بد حال توجه می شود. از اینرو سناریویی که این مهم را برآورده کند، حتی اگر هزینهی زیادتری نیز داشته باشد، نسبت به دیگر سناریوها برتری دارد. با توجه به بهبودهای ناشی از سناریو E، این سناریو علیرغم هزینه بیشتر، به علت اهمیت آن، نسبت به D ترجیح داده میشود. سناریو D نیز با توجه به هزینه کمتر نسبت به دو سناریوی دیگر به عنوان سناریوی دوم بهبود تعیین میشود.
<b>Introduction:</b> Hospital Emergency Departments (ED) face different problems affecting their performance. Deficiency of inpatient beds, inability to access suitable care, noisy and overcrowded places, etc. lead to prolonged waiting time, increased length of stay and low service quality.
The main purpose of this study was to improve patient flow processes in emergency departments.<b>
<br>Method: </b>The population of this descriptive- applied study included unlimited number of patients who referred to the ED of the selected hospital. Colored Petri Net was used to model patient flow processes and CPN Tools software was employed to simulate and investigate different improvement scenarios. Input data for the simulation model were obtained through timing during data collection period. Patients’ waiting time, length of stay, and resource utilization were defined as performance measures. Four improving scenarios (B, C, D, and E) were defined and their effects were investigated to improve processes. <br><b>
Results:</b> According to simulation outputs, scenario B led to 45% decrease in waiting time for the specialists while the other scenarios had no positive (decreasing) impact on waiting time. Scenario E was the most effective one in decreasing the length of stay for patients at different ESI levels. Scenarios E and D had positive effect on resource utilization.
<b><br>Conclusions:</b> Selection of the best scenario depends on the mission of ED, i.e. to save patients’ life. Therefore, a scenario which is the most effective one in saving life must be chosen even if it is expensive. Although it is expensive, scenario E is preferred to scenario D due to its improved outcome. Scenario D is the second best scenario, compared to the other two scenarios, because it is less expensive. <br>
