Investigating the level of observance of standard in medical records of hospitals in social security fund, Tehran, 1997 and proposing a plan for evaluating medical records\' section

This paper is the result of a research by which the level of observance of standards in medical records' section of hospitals managed by social security fund in Tehran, Iran is assessed based on evaluation criteria of American Medical Record Association and also based on national evaluation criteria. It is descriptive study and data are collected by questionnaires, check lists, interviews and observations. Data are analyzed by descriptive statistics. The results of this study indicated that the level of observance of standards in medical records' section in investigated hospital in 1997 has been 34.1% based on evaluation criteria of American Medical Record Association and 59.7% according to the national evaluation criteria. Finally, in order to evaluate the medical records' section in healthcare centers, a plan is presented including 10 separate questionnaires containing the sum total of 1129 questions.

Documentation and its importance in medical records

First we express the importance of writing from the viewpoints of religious leaders, and then we express the importance of documentation especially in medical sciences. The Holy Prophet of Islam said about the value of writing: if a theist dies and leaves only one scientific written sheet, that sheet will protect him/her as a shield from hell. Considering this saying, we see that how important and valuable is the documentation and writing scientific text that it is called a shield. Muslim physicians must carefully keep the medical information of the patients besides treating them in order to do their religious duty. If we consider the history of medical records, we find out that it has existed simultaneous and in parallel with medicine and fortunately Muslim physicians were pioneers. Razi was one of those physicians (Hippocrates before Razi has some narrative and medical reports about the patients) who has considered recording patient information in 1000 years ago like present world hospitals in his arrival before his treatment. An investigation of employment status for graduates of medical records of faculty of management and medical information In Isfahan (1991-1996)

Investigating the employment status for graduates of universities is very important matter. This investigation is important from two viewpoints: first is the positive effect that employment in high percent can make on the graduates of a specific course and second benefit is that determining the employment status of graduates may affect managers and planners in their executive and educational decisions.
Methods: the results were obtained based on a descriptive study conducted in 1997 on 101 form 142 graduates of medical records in Kardani degree. The subjects were graduated in 7 periods (5 morning periods, 2 evening periods) from faculty of management and medical information of Isfahan.
Results: results extracted from questionnaires filled out by 101 graduates of medical records course were as the following:
- 89.9% of graduates had applied for passing their compulsory period.
- 81.4% of graduates had passed their compulsory period or were passing it.
- From remaining 18.6%, 29.4% were continuing their studies in bachelor degree and 70.6% were passing their military services or were activating in occupations other than medical records.
- 50% of those who had passed their compulsory period, were activating in occupations related to medical records as the following:
a. 25.7% as employed under 1-year contract
b. 23% as employed under 5-year contract
c. 20% were employed as official- pilot employee
d. 25.7% were employed as official-final employee
e. 5.6% were employed in another forms
From the questionnaires completed by graduates, required information associated with major and sub-major aims and research questions were extracted and analyzed and the following results were obtained.

Investigating the methods of employees\' evaluation and proposing an appropriate method for evaluating medical records\' staff and suggestions for improvement

Evaluation of employees' performance is some official implementations in order to judge about the performances of working people in an organization in a certain period which includes all behaviors of employees in that period. Performance evaluation is one of the effective instruments in managing human resources and knowledge of supervisors of medical records about the scientific methods of it is necessary for suitable organizational efficiencies. This study introduces different methods of employees' evaluation as "method of performance management" as a suitable method in evaluating the performances and finally makes some suggestions in improving the process of evaluation. We try our best to employ scientific and appropriate methods in evaluating the employees and improve the level of services provided by different sections activating in medical records courses.

Organizational leadership in the old and common format is terminated and it is emphasized that evolution and novelty, creativity, motivation and proper leading of employees be considered as significant properties of present management. Futuristic and evolutionary leadership is proposed as the major factor in the success of present organizations particularly health organizations and it is dared to say that the survival of organizations is related to their leadership. According to the present circumstances, the leaders should be able to employ suitable methods and make the employees successful. Organizational leaders might not satisfy with the current circumstances and try to empower their employees and direct them towards a desired future. So, the leaders must have so influencing character that enable to direct the employees willingly towards the purposes.

Educational planning is a major element for establishment of an efficient educational system. It is in the need of a set of information which is collected from different sources. Parts of this information are provided for planners through the feedback from students about their expectations, programs and educational activities of the faculty and the rate of success that students achieve. The comments of students can be useful not for planners but for the authorities and those involved in education and academic members in terms of their role in education process. This article investigates the opinions of undergraduate and graduate students of health services management about the content of educational plan of the course and its concordance with the required capabilities of graduates in fulfilling their professional responsibilities.

Developments in hospital infection control programs

Hospital infections (NOSOCOmial infection) encompass 5-10% of patients in hospitals which serve critically ill patients with acute diseases. Thus, two million people are infected by them in the country each year. The most important hospital acquired infections are: urinary tract infections, pneumonia, and wound infections. In an article in 1973 in Nebraska journal, the necessary requirements for controlling hospital infections were introduced. There are many alterations in recent decades in this matter and the present study will discuss the developments in infection control in hospital.
