بررسی وضعیت تکمیل رضایت نامه های بیماران بستری در سه مرکز آموزشی درمانی دانشگاه علوم پزشکی ایران
A survey on completeness of inpatient informed consent forms in the three treatment-teaching centers in Iran University of Medical Sciences (IUMS)

<p><strong>مقدمه:</strong> یکی از اساسی ترین حقوق بیماران، رضایت دادن به قبول یک روش معالجه است. همه ی مراکز مراقبتی ملزم به اخذ رضایتنامه ی درمانی از بیمار یا قیَم وی می باشند. علی رغم اهمیَت زیاد فرم های رضایتنامه ی آگاهانه، در تکمیل این فرم ها دقت لازم اعمال نمی شود. با توجَه به اهمیَت فرم های رضایتنامه ؛این مطالعه برای تعیین میزان تکمیل عناصر اطلاعاتی موجود در فرم های رضایتنامه انجام می شود. </p><p><strong>روش پژوهش: </strong>طی یک پژوهش توصیفی- مقطعی؛ و از طریق پرسشنامه میزان تکمیل عناصر اطلاعاتی موجود در فرم های رضایتنامه ها؛ این پژوهش در سه ماهه ی اول سال 1382، در سه مرکز درمانی دانشگاه علوم پزشکی ایران انجام شد. بدین منظور با استفاده از روش نمونه گیری طبقه ای از هر مرکز درمانی، 333 پرونده به صورت تصادفی انتخاب شد. روش جمع آوری داده ها شامل مشاهده ، و بررسی فرم های رضایتنامه بود. به منظور تحلیل اطلاعات از آمار توصیفی(میانگین و انحراف معیار و فراوانی) و نرم افزار SPSS استفاده شد. </p><p><strong>یافته ها:</strong> یافته های این مطالعه نشان دادکه در 2/7درصد ( 24 مورد) بیماران فرم های رضایتنامه آگاهانه و 5/71درصد (234مورد)در فرم ها به ترتیب نام و نام خانوادگی و آدرس کامل بیمار ثبت نشده بود. 2/1درصد (4 مورد) فرم ها فاقد هر گونه تایید رضایت (چه از طریق امضا یا اثر انگشت) بودند. در4/5درصد (18 مورد)فرم ها خط خوردگی داشتند. در 4/2درصد (8مورد) بیماران علی رغم نداشتن سن قانونی رضایت داده بودند؛ و در 6/3درصد (12مورد) توسط افرادی غیر از بستگان سببی و نسبی تایید شده بودند.در 6/87درصد( 74مورد) در مواقع اورژانس فرم ها دارای نام و نام خانوادگی بوده در صورتی که در موارد غیر اورژانس این میزان به 7/94درصد (235)رسیده است. همچنین در 6/77درصد (66)موارد اورژانس، رضایتنامه ها به تایید شاهد رسیده در صورتی که این میزان در موارد غیر اورژانس به 9/59درصد (147)بوده است. در2/96درصد (76) کل پرونده های بررسی شده دارای فرم دیگری به نام فرم برائت بودند. </p><p><strong>نتیجه گیری:</strong> با توجه به اینکه هدف از اخد رضایت، کمک به بیمار جهت اخذ بهترین تصمیم بوده و با توجه به اهمیت فرم های رضایتنامه برای بیمار و مراکز درمانی، لازم است تا کارکنان بخش پذیرش (که وظیفه ی تکمیل این فرم را دارند) در تکمیل این فرم ها نهایت دقت را مبذول دارند. آموزش کارکنان بخش پذیرش در زمینه ی اهمیَت تکمیل ؛ و عواقب ناقص بودن این فرم هالازم است. </p>
<p><strong>Introduction:</strong> To take informed consent for treatment is one of the essential rights of each patient. All healthcare centers must complete consent forms. In spite importance to get informed consent forms, there isn't adequate paying attention to complete these forms. This research is aimed to review the completeness of consent forms in IUMS's teaching centers. </p><p><strong>Methods:</strong> In a descriptive, cross-sectional research we reviewed the completeness of consent forms using a questionnaire. We selected 330 consent forms of inpatient patients in treatment-teaching centers from IUMS by a stratified random sampling method. Data were collected through observations and consent forms survey. In order to providing information we used descriptive statistics by SPSS.<strong> </strong></p><p><strong>Results:</strong> Our findings showed that there were incompleteness for patient's family and first names in informed consents forms in 7.2% (24 patients), and for of patient's addresses 71.5% (234 patients). There weren't any sign or finger print in 1.2% (4) forms. In 5.4% (18) forms was bad handwriting. 2.4% (8 patients) patients didn't have legal age while they singed form consent, and 3.6% (12 patients) forms singed by consanguinity and casual relatives. In emergency conditions there were for 86.6% (74 patients) family name and first name recorded while in non emergency conditions 94.7% (235 patients) were recorded. Also forms were confirmed by witnesses in 77.6% (66 patients) in emergency cases while in non- emergency cases it was 59.9% (147 patients). It was applied another form as acquaintance (exemption) in 96.1% (76patients). <strong></strong></p><p><strong>Conclusion:</strong> Goal of getting consent forms is providing the best decision-making for patients. Because importance of getting consent form for patient and treatment centers, it is necessary to reception personnel training to acquire knowledge about consent forms and their completeness. </p>
