The Holy Prophet of Islam said about the value of writing: if a theist dies and leaves only one scientific written sheet, that sheet will protect him/her as a shield from hell. Considering this saying, we see that how important and valuable is the documentation and writing scientific text that it is called a shield. Muslim physicians must carefully keep the medical information of the patients besides treating them in order to do their religious duty. If we consider the history of medical records, we find out that it has existed simultaneous and in parallel with medicine and fortunately Muslim physicians were pioneers. Razi was one of those physicians (Hippocrates before Razi has some narrative and medical reports about the patients) who has considered recording patient information in 1000 years ago like present world hospitals in his arrival before his treatment.
